All three requirements must be met before a student can practice/play in a school sport.

The participation fee for each Middle School activity is $75/activity with a family cap of $225 (Trapshooting, Robotics & FCCLA are NOT INCLUDED in the family cap).

PHYSICAL

EVERY CMS ATHLETE MUST HAVE A SPORTS PHYSICAL ON FILE IN ORDER TO PRACTICE. Since 7th grade is the first year our students participate in school activities, ALL 7th graders will need to turn in a sports physical. 8th graders who participated in a school sport as a 7th grader should have a physical on file. If not, they need to turn one in. Check with the office if you are not sure of your child’s sports physical status. Physical Forms can be downloaded below.

All extra curricular athletic activity  or other activity will now need to register on-line. You will need your students 6-digit student ID number, otherwise known at the PERM ID number or your Family Key. Please follow the directions on the website listed below.
